What happened?Some servers hosted by IST experienced an interruption in service due to a failed Storage Area Network (SAN) switch change.

What was timeline of the incident?Start: 2014/02/12 01:00 – IST support analysts discovered an issue with the SAN change which was affecting servers.2014/02/12 01:05 – IST support analysts began working on the issue.2014/02/12 04:00 – IST support analysts began to identify and restore affected services.2014/02/12 14:15 – IST support analysts continued to monitor for, and repair affected services.2014/02/12 14:50 – Service was restored for Ancillary Services users. IST began verifying that all affected services had been restored.End: 2014/02/12 16:58 – Service was confirmed to be restored.
